NOTE: Level 3 plans are for San Antonio employees only.  Employee must be enrolled for at least 3 months before any plan changes. All enrollment cancellations must go through HR department .

If you choose to participate: Please note that ProScribe will deduct the discounted membership rate from your paycheck bi-monthly, with the exception of the first and last month's fee which will be deducted from one paycheck.
