IMPORTANT NOTE: If your FTE is below 24 hours per week, you are currently in PRN status. You are NOT eligible for the payroll deduction program. Please visit your nearest Gold's Gym with proof of employment to enroll. DO NOT SUBMIT THIS FORM.
